Define required training and frequency, then develop or identify sources of staff training for:


ü      Intake

ü      User records creation and maintenance

ü      Matching user to appropriate AT

ü      User instruction in device use (technical assistance)

ü      Web site operations

ü      Delivery/Pickup of equipment

ü      User follow-up

Prior education, training or specific credentials required

Specify for each position in hiring job description

[HR should verify]

HR verifies education and credentials during hiring process, maintains log of required updates

After-hire staff training, OJT, training leave and reimbursement

Specify for each position skills that may be required within given time period; whether OJT will be provided or from external source; list approved sources of external training; document procedures for reimbursement

Document process for acquiring internal or external training and reimbursement (if applicable)

What volunteers are permitted, not permitted to do

Approval process for volunteer to perform tasks

Document forms, approval process

Consequences for failure to acquire or maintain required levels of training or credentials

When, how policy is communicated to employee

Document process for ensuring maintenance of credentials, notice that continuing education is required

Process for imposing consequences
